Do I Need Filler, or Would a Skin Treatment Be Better?

One of the most common questions I hear in clinic is: “Do I need filler, or is there something else that would suit me better?”

The honest answer is that it depends on what you would like to improve. Filler can be an excellent treatment when used carefully and for the right reasons, but it is not the answer to every concern. Sometimes the best result comes from improving the skin itself. In other cases, a small amount of structural support alongside a skin treatment may create the most natural-looking outcome.

Start with the concern, not the product

Rather than choosing a treatment because it is popular online, I prefer to begin with what you are noticing when you look in the mirror. Is your skin feeling dry or dull? Have fine lines become more visible? Do your eyes look tired even when you are well rested? Or have you noticed a loss of support or balance through the cheeks, jawline or profile?

Once we understand the concern, we can decide whether the priority is skin quality, muscle movement, volume, structure—or a combination of these.

When a skin treatment may be the better choice

If your main concerns are dullness, dehydration, fine texture, enlarged pores or a general loss of glow, adding volume is unlikely to be the first step. Treatments designed to support skin quality may be more appropriate.

Depending on your skin, this might include skin boosters, polynucleotides, microneedling or a carefully selected chemical peel. Biostimulating treatments such as Sculptra or Radiesse may also be considered when the aim is to encourage gradual collagen support rather than create an obvious change in facial volume.

These treatments work in different ways and results are not instant in every case. Some are best carried out as a course, followed by maintenance. Your treatment plan, expected downtime and likely timescale will always be discussed during consultation.

When filler may be helpful

Filler remains a very useful treatment when there is genuine volume loss, a lack of structural support or an area of facial imbalance. Used precisely, small amounts can soften a hollow, support the mid-face, improve profile balance, define the jawline or create a subtle and natural-looking lip result.

The modern approach is not about changing every feature or chasing an overly filled appearance. It is about assessing the face as a whole and making small, considered adjustments where they will have the greatest benefit.

The difference is that we now have a much wider range of treatment options, so filler no longer needs to do every job. Combining it thoughtfully with treatments that support skin and collagen can often produce a fresher, more balanced result with less product.

What about lines and ageing well?

Lines can develop for several reasons. Some are caused mainly by repeated facial movement, some become more noticeable as collagen and hydration reduce, and others relate to changes in facial support.

That means the most suitable option may be an anti-wrinkle consultation, a skin treatment, a collagen-stimulating treatment, filler—or a carefully planned combination. The goal is not to remove every line or make the face look frozen. It is to help you look rested, healthy and like yourself.

A personalised plan can use more than one approach

Many people do not fit neatly into a single treatment category. For example, someone concerned about the eye area may benefit from polynucleotides or another delicate-eye treatment to support skin quality. If there is a suitable hollow, tear-trough filler may also be considered. A course of treatment is often recommended, particularly when the aim is gradual skin improvement.

Likewise, concerns around the jawline and neck may involve skin laxity, collagen loss, muscle activity and facial proportions. Treating just one of those factors may not give the most balanced result.
